I got one of these today. I've been using Stolen Artistry to farm nightmare fuel components a couple times a day for over a week now, but this is the first time a sinew has dropped. (I only have one more in inventory, plus a ghoulish crown; I think both of those are from an earlier reset though I'm not sure.)

This was completely unexpected, but it does let us narrow down requirements:

Skills: I forewent all the ghoul king skills for Vassal of the Seas
Effects: My only effects were Ocean Sight and Sparkling Blood
Equipment: red goggles, charred head, shock shield, fused breastplate, fused plates, sampling kit
Techniques: I used two attacks. Didn't think to note which, but the ones in my deck are Tesla Coil, NitroX, Molotov Cocktail, Nitro

Odd that both Arkaim and I were using the charred head and fire techniques, but given that the same setup didn't result in sinew the first 6+ times, I suspect high perception might be the most vital component. I recently reached 50 base, which could be significant I suppose.

I got it without having Trauma Tech, Sashimi Knife, or using Ghoul Bite and Call to Feast. I had the charred head and used fire techniques.

I was etheric when I got it, though. (Kingly Favor I have but I doubt that has anything to do with it)

I did basically everything possible in The Black Sketchbook. Shooting up everything I could and whatnot.

Got two of them in three one-ghoul combats with Kingly Favor, Ocean Eyes, a Myers sampling kit and no tome of binding.

I would also not be surprised if it depended on how you resolved the ghould king thing (i.e. if you have ghoultouched, born of sorrow or kingly favor)

Al, Mag, and I have all had trouble getting these.

I'll try the tome of binding tomorrow, since I met all the other requirements but failed to get any sinew in 8 goes. Doesn't make sense to me though. EDIT: re below, I have Born of Sorrow.

Other possibilities:
Trauma Tech? (knowledge of anatomy)
Sashimi Knife? (cutting tendons)
killing with Ghoul Bite / Call to Feast?

I'm pretty sure all the following must be true to get sinew:

-Must have completed the Black Sketchbook with the Grisly Crown
-Must be Etheric
-Must have Myer's Sampling Kit
-Must not have the crown equipped, but you can definitely have it in inventory (?)
-Must have the Tome Of Binding equipped (???)
