For the centipede - Argeth reports the broken ankle result with 61 reflexes. I got the avoiding broken ankle with 49 reflexes. I had cyberlegs installed if that matters.

I think wearing the Midgard security disguise may block the Midgard Security combat encounter here.
After killing appox 5 inspectors, I got the roiling dust drone fight, and Emergency Situation choice very next turn. Gonna keep better count of the numbers tomorrow. Construction Crew+Shovel Drone fight seems to be also at about this alert level.
I think Cracked Ankle may be checking if you have enough evasion power active. I've needed at least 3 evasion power to avoid getting the Cracked Ankle result.

Having all four pieces of the third eye hunter outfit here does not prevent the third eye ganger x2 from attacking you (from completing Oldtown Challenge). It also doesn't prevent the Authorized Personnel Only or Lot Inspection encounters, nor change results for threatening in Authorized Personnel Only.
