For what it's worth, here's the game text you get when you learn the skill:

After the combat, you patch yourself up, healing 1 hit point.

Your practice with the Trauma Reference has given you some excellent insights into first aid as a whole.
You've learned a new Skill: Trauma Tech

I had the trauma reference equipped and no other first aid items or effects. Maybe you have to heal a certain number of HP with the reference equipped, but I suspect it's just a random roll each time you heal with it equipped.

could be. I finally got it with both trauma reference and biomonitor kit equipped. Extra First Aid might help?

Might it be getting injured in a ton of ways with the trauma reference equipped? Kinak seemed to maybe imply it was some sort of sequence, though, and that you could get off track? I seem to recall getting the skill while messing around in the Damaged Buildings.
So, maybe healing after combat after getting one of the following:
Animal bites, fire, and radiation burns? Okay. Sucker wounds, "etheric intoxication," and "green fire"

Animal bites would be from twisted hounds. Fire… could be quite a few things. Radiation, I'm not sure.

Sucker wounds, fight a tentaclemonster, slags poisoning, and green fire being the spell.

I guess we could spade this by having people who reset equip the book and try to do each of the things once? Or definately, if possible, get good logs of the battle you got the skill on to see if any of the attacks seemed relevant?

Just podcast-listened/fished/first-aided for quite a bit at the Docks, I'm up to 316 hp healed from first aid if I've counted right. I'll get back to this and keep doing this until a reasonable point (1,000?) to determine whether you can get the skill from pure hp healed.

EDIT: 370 hp, but I just got to the point in the podcast where he did say it was somewhat complicated (surprise, right?). So I guess this straightforward approach is already disproven.

I'm pretty sure it's not related to the amount of HP healed. I had the trauma reference equipped, and adventured for quite some time in the slags, and got the following message after a fight with an eye:

Your practice with the Trauma Reference has given you some excellent insights into first aid as a whole.

You've learned a new Skill:  Trauma Tech

After every combat, the reference healed me 1-2-3 HPs, and I can't have had 300 points of healing accumulated…

Once I concretely decided to go for this, I went up and down the damaged buildings etheric and not etheric, wandered about in the happy hour and summoning labrynth beasts doing other things, then fished at the tainted shoreline (etheric) for quite a while. Finally got the skill healing 2 hp after a typical combat versus an umpteenth writhing mass. Hm.

Mag suggests that it could be healing X damage from each source alluded to in the description. In which case writhing masses, the lowest damage dealers by far in my case, would certainly be the last one to meet the requirement.

Data point: I got this after fighting a waterlogged corpse (unsewn), healing 1 HP at the time.

Data point:

+ show block

Spent about 2 days killing stuff on the docks, shoreline, and 1st floor. About 50/50 etheric.

You punch the writhing mass, throwing the whole weight of your body behind it and dealing 30 damage.
The writhing mass grabs you with its tentacles, but you manage to fight them back.

You viciously slash the writhing mass for 29 damage.

You latch onto the writhing mass and tear a chunk out of it with your teeth, dealing 32 damage.

You've won the battle!

After the combat, you patch yourself up, healing 2 hit points.
Your practice with the Trauma Reference has given you some excellent insights into first aid as a whole.

You've learned a new Skill: Trauma Tech

You've earned 1 XP in Reflexes

You've earned 2 XP in Strength

Kinak says: "Hmm, let me check. I'm pretty sure it's at most semi-random. Hm, nope. It's totally not random at all actually. 0% randomness. Nada. Warning: Things in games tend to be closed systems, and trauma tech is a lot more complicated than that. But not in a soul rending kind of way (probably). Going through a couple runs, just spading it, may very easily result in you finding out the answer painlessly enough. [laughter] But if you misstep, there are some pretty serious monkey wrenches that might show up. But still not random. But there's a lot going on under the hood, not all of which will jump out at you."

After spending the better part of two days adventuring before finally getting this skill, I have a theory of sorts. First of all, I kept the trauma reference equipped the entire time, in case unequipping it resets whatever counter(s) it uses (like the HKGames helmet and the feathered cloak). I finally got the skill from a nondescript fight, but only after I had finally manged to get a Midgard SD1 to hit me with a Microrocket, a fire technique. So my guess is that first you need to be hit with one of each kind of attack with the reference equipped, then a counter starts running. At that point, healing X HP or after X fights or something gives you the skill. But until you get the counter running, no amount of healing will do it (unless Kinak, in his mercy, put in an alternate trigger too).

I'm inclined to think that having a Writhing Mass attack you with a tentacle or getting the Slags Poisoning effect isn't required, since on a podcast Kinak said the skill was gettable when the trauma reference rolled and that was before either of those things were encounterable in-game. But I wouldn't be surprised if having bad effects like Broken Bone, Coughing Fit, and the like counted as kinds of trauma as far as hitting some hypothetical "experience with enough different varieties of trauma" threshhold.

My working theory is that you require:

• fire damage
• ghoul bite
• withering mass
• slags poisoning
• etheric
• being hit by an etheric attack
• Some amount of damage healed

I'm going to try logging, see what happens.
