I dived into the lake with only 2 Fire Defense. Got two items and only took 3 damage.

I am pretty sure that the amount of Fire Defense you need and how much damage you take depends on how many of the five black stone walls you've destroyed in the Dry Riverbed.

Erm, there are no dry riverbeds or black stone walls Below the Charnel House. Are you sure you didn't confuse A Lake Covered By Mists with Lake of Fire? I admit they are similar.

I'm amazed how even the drop rates turned out combined

No Fire Defense Loot

Beaten to hell loot:
holocaust coat: 6
etheric focus: 8
sulfur: 13

Survival Loot:
holocaust coat: 4
etheric focus: 2
sulfur: 2

Loot Combined!:
holocaust coat: 10
etheric focus: 10
sulfur: 15

The Survival and Beaten to hell loots are lobsided enough that I wonder if surviving the experience or not really does affect your chances.

Nothing special for diving into the lake with paper mask and paper mache horn weapon (nothing burns up).

The option to fish does not appear with a hooked chain, even if you have Fishing skill (unlike the Lungs of Lead encounter).
