It seems very likely that the Chamberlain or the scroll of lineage would be sources of an unlockable title. Any other ideas?

That was my first thought too. Though I would imagine that titles would be unlocked for things that are not currently given other rewards and we have an avatar from the Chamberlain. The doing all 4 survivors quests conduct gives a crown too. Does the sea king knight you? We'll see once we find a few what kinds of things they could be.

EDIT: Beating someone at chess or pool?

So, here's the list of conducts that I can confirm do not have titles associated with them:

David
Hel
Lo
Dr. Amundsen
Overwhelming Evidence
Extreme Diet
Old-Fashioned
Eclipse Free
Flesh and Blood
Indirect Fighter
Assassin
Bruiser
True Immortal
Gang Warrior

And conducts I can't say for certain on, but based on others it seems unlikely they have titles:
Just Say No
Full Body Warranty
Eclipse Purist
Etheric Master
Firestarter
Gunslinger

Most of the rest are up in the air. A lot of the stuff that falls under 'Miscellaneous' conducts seems pretty plausible for titles.

I just got Array title. I only have the Best David and Best Lo so far. It's not clear to me what triggered this.

So, I got the Holiday Headhunter title this year, when I have not in the past despite beating the manyheaded snowman. I think the difference this year might be one of two things. I had one fight in which I managed to get 5 floating heads, and I had another couple fights in which I managed to kill the snowman without knocking off any of its heads. I'm not sure on the former, but I know I didn't do the latter on previous years.
