With a normal shield equipped	-	4 (M,R)
You raise your shield to block. <Opponent attacks>, but your shield absorbs all but <X> / of the damage.
With an improvised shield equipped	-	4 (M,R)
You awkwardly raise your improvised shield to ward off attacks. <Opponent attacks>, but your 'shield' absorbs all but <X> / of the damage.
With a shock shield equipped, vs. Melee	3 (Untyped*)	4 (M,R)	* Damage increases with Evasion Power, but opponent's defenses do not reduce it.
You brush <opponent> with your shock shield, dealing <X> damage. <Opponent attacks>, but you ward off all but <X> / of the damage with your sparking shield.
With a Midgard blastshield equipped	-	4 (M,R,F)
You duck behind your blastshield. <Opponent attacks>, but your blastshield absorbs all but <X> / of the damage.

With a shield of scales equipped	-	6 (M,R,E)	Gives Sudden Bash if it chimes or Striking Shield if it shimmers
You raise your scaled shield to block. <Opponent attacks>, but your shield absorbs all but <X> / of the damage. Or occasionally against melee: <Opponent attacks>, but your shield absorbs all but <X> / of the damage. It chimes like a struck bell. Or also occasionally against etheric: <Opponent attacks>, but your shield absorbs all but <X> / of the damage. Its scaled patterns shimmer with green energy.
With a shield of scales equipped, following Etheric Striking Shield	-	10 (M,R,E)
The serpents surround you, hissing defensively. <Opponent attacks>, but a serpent takes the blow, absorbing all but <X> / of the damage.
With a shield of scales equipped, following Etheric Serpent Pattern-Striking Shield	-	12 (M,R,E)
The serpent coils around you, shielding its master. <Opponent attacks>, but the serpent blocks all but <X> / of the damage

Sources

Avoiding damage using Dive for Cover with a shield or improvised shield equipped, some of the time. Semi-random or unknown factor? Seems to unlock more often if you block large amounts of damage with Dive for Cover. Eg. Blocking 1-2 damage, even repeatedly, does not ever? give anything, but blocking 6+ damage does often.
